The state of Florida has six (6) district offices and six (6) delegated agencies. Choose and contact the office that would regulate permitting for the county of the application.  The six (6) delegated agencies are for Duval, Sarasota, Hillsborough, Palm Beach, Broward and Dade counties.

Florida D.E.P. Wastewater Permit Application Forms
The permitting of wastewater pump stations and collection and transmission systems will require the submittal of the proper D.E.P. forms. Each application must be submitted on D.E.P. form No: 62-604.300(8)(a). Each application must also have
D.E.P. form No: 62-604.300(8)(b) submitted upon project completion.
